Pre-appointment details
Prior to your scheduled appointment, Eye on You will send you a pre-consultation questionnaire via email. This questionnaire aims to gather essential information to better understand your needs and tailor our services accordingly.
We kindly ask that you complete and send back the questionnaire before your scheduled appointment. This ensures that our team has ample time to review your responses and prepare adequately for our meeting.
Cancellation policies
Cancellations made at least 24 hours before the scheduled appointment will receive a full refund of any deposit paid.
Cancellations or rescheduling within 24 hours of the appointment will result in forfeiture of the deposit.
